Community Healthcare Assistant

We are looking to recruit an experienced and enthusiastic Healthcare Assistant who will support the Community Nursing in Middlesbrough, Within Greater PCN, but will also be part of the larger Tees teams, within the Community Care Centre.

To provide a high standard of health care to patients within their own homes, as part of the integrated community nursing service the post holder will contribute to the admission avoidance agenda by providing high quality care to patients.

He/she will be responsible for carrying out designated work, much of which will involve direct patient care. This care will be delegated by trained staff and the post-holder will be expected to work under the indirect supervision of trained staff. The post holder will work in a person centred way to achieve clear outcomes and adhere to trust and team objectives.

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills. You will be expected to attend the New and Aspiring leaders programme, and the Foundation Quality Improvement Programme that the Trust delivers. After you complete this one and a half day of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses and bitesize programmes and leadership apprenticeships at level 3,5 and 7.
