We are a four-partner, training and dispensing practice, working from modern purpose-built premises in Faringdon, Southwest Oxfordshire with a growing patient list.

We have a fantastic opportunity to join our lively and welcoming team of Care Navigators. As a Care Navigator you will be responsible for ensuring that our patients are assisted to access care within our own multidisciplinary team, as well as wider NHS services. You will also periodically work alongside our triage GPs to ensure that patients are effectively and safely triaged each day. Successful applicants will find the role widens their knowledge and understanding of triage processes, local services, and patient care. Please see the job responsibilities below for more information.

We have opportunities for both permanent full and part-time positions. £10.50 post probation.

**Responsibilities**:

- Providing a friendly and caring service for our patients
- Signposting to local and wider NHS Services
- Working with clinicians and the multidisciplinary team to provide efficient patient care
- Taking calls and assisting patients to access the triage system
- Booking patient appointments and rebooking as required
- Processing requests for online access
- Assisting in patient recalls
- Responding to patient queries via Telephone, E-mail, and the reception area
- Checking patients in and keeping the reception area organised and patient friendly
- Handling patient data, documents, and information in a confidential manner
- Working alongside triage GPs to signpost patients and book appointments appropriately

Negotiable hours, part and full-time positions available.

Monday - Friday

07:45 - 18:30

**Job Type**: Part-time

**Salary**: From £10.50 per hour

To keep Pt Services safe we have a screen around reception, screens between desks, and masks are worn at all times. We remain Covid-19 Compliant safe throughout the practice.

- Working in a GP surgery: 1 year (preferred)
