Medical Coder/summariser

Reviewing new and existing patients medical records and inputting, updating and tidying an accurate summary of the patients medical history. Read coding a variety of information into the practice clinical system including checking medication, populating templates and coding from letters and pathology. Recording of A&E attendances onto patient records. Deal with queries relating to coding. Recording vaccinations. Identify relevant clinical information contained in patient correspondence. Processing workload for new patients links, GP2GP integration and incoming medical records. File and code screening results bowel screening, retinopathy, and bone density, smears including templates completion and passing information to nursing team where necessary.
