The post of Practitioner is open to qualified staff from the following disciplines RMN, Occupational therapist, or Social worker Practitioner. We are expanding our mental health crisis support services within SCAMHs and developing a new supportive emotional wellbeing HWB for our client group based in Carmarthen. The Crisis team operates on a 24hour seven day service. We provide mental health assessment and support for young people who have either come to the service as a result of crisis or who need support or therapeutic intervention in the community. We are developing a new emotional support crisis service during 2023 for a supportive HWB which will will provide a safe place for Young People who are experiencing periods of emotional distress and require a safe place where they can be supported to de-escalate their emotional distress before returning to their home environment. The post of Mental Health Practitioner will work across the three counties and therefore staff will be covering the main Crisis team and also allocated duties within the supportive HWB.

Role includes working with Qualified Practitioners and support staff undertaking initial MH assessments within A&E, or following up urgent referrals. A key role will be liaising with colleagues in adult service to help facilitate admission for Children and YP to age appropriate beds identified within HDUHB Paediatric ward at GGH and the acute adult mental health admission ward. You will undertake follow up visits within YP home and community settings providing therapeutic in supportive visits to the YP on the CATT caseload.

Autonomous practice will be to undertake home visits for YP to support them during crisis period and develop therapeutic interventions and liaising with the YP and their families as means of help individuals manage their recovery from their initial CRISIS intervention.

As Qualified staff members of the CATT team you will be responsible for allocating roles within the team, undertaking assessment within secondary services, using the Mental Health Measure Wales and responsible for developing initial assessments, care and treatment planning( CTP) and WARRN risk assessment and working with other members of the wider multi disciplinary teams.

**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire,

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres.

Numerous locations providing mental health and learning disabilities services.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

The development of the new supportive HWB is the first of its nature in Wales. It requires Practitioners to take a lead in delivery of this service to support YP through periods of emotional distress, develop positive coping strategies and build resilience in order to remain safely in the community.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
