**Care Navigation is a critical front line role in delivering care to patients in the most effective way possible. A trained Care Navigator is so much more than just a Receptionist. The role requires interaction with patients, clinicians and other care services. It is both challenging and rewarding to know that this pivotal role makes a difference.**
**Main duties**:
**Reception**
- Checking-in patients who are consulting with members of the practice team
- Open and lock up practice premises and maintain practice security.
- Dealing with patient enquiries at the counter
- Assisting patients in the use of the blood pressure assessment equipment
- Dealing with registrations and deductions of patients and their records
- Make sure that the waiting room kept is clean and tidy throughout the day, with relevant information posters and digital displays maintained.
- Ensure that any tasks requested by the clinical team are carried out including informing patients of their results and arranging appointments.
- Undertake administrative duties as requested by the Team Lead
**Telephone & Communication**
- Handle all telephone and general enquiries in a professional manner, referring urgent calls to clinical staff.
- Using a telephone triage approach to navigate patients to the appropriate service or clinician.
- Make appointments for patients in accordance with practice protocols
- Ensure that messages and requests for visits are recorded and that the appropriate clinician receives them in accordance with practice protocol.
- Liaise with hospital departments and community staff
**IT SKILLS**
- Ensure all Tasks and Notifications received via S1 are appropriately completed
- To ensure that patient confidentiality is maintained at all times
- Maintain the accuracy of patient records to ensure that all relevant clinical information is accessible by clinicians
**PERSONAL RESPONSIBILIITES**
**Personal/Professional development**:
- **Take personal responsibility to maintain awareness of all updates to clinical pathways and protocols, practice policies and procedures.**:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of their own personal and/or professional development.
- Completion of all mandatory and supplementary training as requested by the Reception Team Lead or Practice Manager within agreed timeframes.
**Other Tasks**
- Attend regular Reception/Admin Team Meetings and Staff meetings as required
- Any other tasks or duties allocated by Team Leads, Deputy Practice Manager, GP’s or Practice Manager
**Salary**: From £11.79 per hour. The salary will be reviewed upon the successful completion of a 6 month probationary period. (Any adjustments to National Minimum and Living Wage values which occur during the probationary period will be reflected in adjustments to salary )
