Band 4 - Health & Wellbeing Hub Coordinator - 30 hours per week - 12 months fixed term

Are you ready for an exciting challenge? The new Lillington Healthcare Centre has created an opportunity to develop a Health and Wellbeing Hub in the heart of the community.

The post holder will support the Trust’s ‘Big Move’ on prevention, tackle health inequalities and support people to live healthy, happy lives. The Health and Wellbeing Hub will offer and link to a range of preventive services that take health and wellbeing services out to the public and support people to remain well in their communities.

The Hub will encourage community engagement and participation and provide a space within the community to provide information, signposting, meeting spaces and services to empower individual volunteers and community groups to deliver local support.

The Health & Wellbeing Hub Coordinator will play a vital role in the development of the Health and Wellbeing Hub, creating partnerships with the local authority, community organisations, charities, healthcare professionals and volunteers to provide a range of activities and services that support the health and wellbeing of the local community.

The post holder will mobilise volunteers within the local community, developing volunteering roles within the Hub and the local community to support people at all stages of their healthcare journey and respond to local needs.

The Volunteering and Health & Wellbeing Information Centre Coordinator will play a vital role in the effective and efficient running of the Health and Wellbeing Information Centre (HWIC) and Volunteer Service at Lillington Health Hub.

The post holder will supervise and coordinate the day to day running of the Health and Wellbeing Information Centre, assisting and deputising for the HWIC Manager. The role includes supporting the development of volunteering roles and activity within the community and HWIC, interacting with departments and external organisations to ensure all opportunities are maximised.

The post holder will supervise and coordinate volunteer activity and engage with new and existing volunteers on a daily basis. The post holder will also provide effective recruitment, training, support and recognition of volunteers at Lillington Health Hub.

To ensure accessible and comprehensive Health & Wellbeing Information and resources are available in a range of formats.

Coordinate the delivery of prevention advice and signposting, tailored to support community needs and increase the take-up of community and voluntary sector services.

Assist with the development and oversee the implementation of new volunteering opportunities which attract a diverse range of people and are reflective of the individual needs of the local community.

Create links with clinical teams across the Trust to provide Health Awareness sessions and screening clinics to support people to prevent or manage health conditions, prevent deterioration as well as provide key health messages.

Work with a range of partners to understand the needs of the local community and through partnership work oversee the delivery of a tailored offering of ‘Living Well’ Services to promote positive health and wellbeing.

Engage with local communities to help address barriers to accessibility and health inequalities, developing partnerships with voluntary sector groups that can share targeted health information messages.

Facilitate regular ‘pop up’ events linked to local Trust priorities and national events and initiatives.

Create a timetable of activity/ ‘what’s on’ at the hub and actively promote this within the community.

Support the development of an outreach strategy to increase participation and engagement in hub activities.

Coordinate the development of marketing materials, campaigns and social media to showcase the activity within the Hub, providing clear information to all stak
