Health Improvement Worker

To support and engage with people with Serious Mental Illness to have their cardio-metabolic risk factors measured. To carry out observations and take blood samples in order for risk factors to be identified. To support patients in understanding their risk factors, how to manage their risk factors and agree a plan with the patient for ongoing monitoring and management of the risks under the supervision of a registered clinician. The Health Improvement Worker will work as part of the Local Mental Health Team and wider PCN to deliver physical healthcare interventions to patients with Severe Mental Illness (SMI).
The post holder will have a specific remit to support achievement of the physical health standards for people with SMI by engaging service users in routine physical health checks and support the person to work towards personal physical health goals in order to improve their physical and mental health. This involves supporting people on the GP Serious Mental Illness (SMI) register living in the locality to have their cardiometabolic risk factors measured. They will be 18+ so may be under the care of AMH, MHSOP or solely the GP.
