**Here’s the role**

We are seeking two Advanced Leaving Care Practitioners in our After Care service. These fantastic new positions provide an excellent opportunity for career progression for Personal Advisers and Social Care staff. The Advanced Leaving Care Practitioners will work directly with young people as they leave Local Authority care and work closely on service development, participation, and practice improvement.

**Key Responsibilities**:

- Hold a small complex caseload of young people who are Care Leavers.
- Provide reflective supervision to Personal Advisers in the After Care service.
- Take on lead responsibility areas such as Participation, Accommodation, Education, and Independent Living Skills.
- Role model good practice to prepare young people for leaving care, providing support with practical, emotional, and social support.

**Key Requirements**:

- The ability to assess the needs of young people and prepare plans for their future
- The ability to work in partnership with other professionals / agencies and build partnerships to improve service delivery
- Understanding of legislation and guidance regarding Children, Families, Children Looked After and Care Leavers.
