**Role: Mental Health Practitioner**

**Key requirements**:

- At least a few months’ MHP experience in primary care over the last few years.
- Ability to establish rapport and build therapeutic relationships with patients
- Relevant qualification
- Ability to follow care plans and treatment protocols
- Demonstrated leadership abilities in a team setting
- Proficient in using computer systems and software for documentation purposes

**The duties** of the role may vary from time to time depending on current and evolving workloads and staffing levels:

- Work with patients to support engagement with appropriate services and treatment, which may include onward referral to secondary mental health services, or sign-posting to other mental health services, voluntary services and support groups.
- To work independently in the Surgery and the Community, providing assessment, treatment and referral of patients with specified mental health problems and unexplained symptoms.
- You will be involved in providing a high level of safe and effective care.
- Keep detailed records of all clinical activity in line with service protocols on the GP record systems.
- Review medication for therapeutic effectiveness appropriate to patient needs and in according with evidence based practice and national and practice protocols.
- Adhere to all Trust policies, procedures and guidelines.
