2 x Band 4 Patient Pathway Coordinators

We are looking to recruit two a well-organised and self-motivated Administrator to support the Child Protection, Child Death Department and General Paediatric Consultants

As the Patient Pathway Coordinator, you will be responsible for undertaking a wide range of administrative and secretarial duties, which include booking child protection, processing child death notifications and general paediatric appointments, minute taking for child death, typing and processing clinical letters, reports, court case summaries and other correspondence as required by the consultants and medical team.

The Patient Pathway Coordinator will lead on the tracking of pathways of patients for general paeds and also to manage the child death and child protection process, by working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national and local targets are met.

This post is required to support a group of consultants based at the Sunshine House, ensuring their reports and referrals are accurately formatted and distributed in a timely manner.

The staff member is also responsible for processing child protection report in a timely manner and submit to the police and Southwark Social Service.

To receive Child Protection Referrals and Child Death Notifications, record accurate details and process in accordance with agreed procedures

To record and register all child deaths on the Patient Electronic System and update progress notes with related information.

To notify community health professionals of the child’s death.

Arrange Child protection medicals in liaison with the duty clinician, parents/carers, Social Workers, police, other agencies as required.

Attend child death meeting, take minutes of the meeting and circulate within agreed timeframe.

Process and action case conference request, minutes and strategy meetings for the Evelina clinical team.

The Evelina London Children’s Hospital is a busy secondary and tertiary children’s hospital. We are committed to excellent patient care and providing a first class service to children and their families.

We provide a comprehensive range of services for the 121,000 children and young people that live in Lambeth and Southwark. This includes universal services - health visiting, family nurse partnership, school nursing, nutrition and dietetics, and a newborn hearing screening programme. Our specialist services cover community paediatrics, speech and language therapy, physiotherapy, occupational therapy, complex needs nursing, sickle cell and audiology. As well as part of statutory services including looked after children, child protection, safeguarding, and adoption and fostering.

We have two main health Centres Mary Sheridan Centre in Lambeth and Sunshine House in Southwark but we have many more across Lambeth and Southwark. We also provide services in schools, community buildings, in families’ homes, and occasionally in hospital. Through group sessions and individual appointments, face-to-face and virtual, we hold over 176,000 appointments every year

To ensure that day-to-day electronic mail is handled effectively on behalf of the child protection department, in line with agreed standards

To act as a central point of contact for the child protection department.

Review and support development and implementation of Standard Operating Procedures.

Responsible for collecting and updating patient information on the relevant databases in line with local policy, to ensure efficient pathway progress. This includes maintaining the accuracy of data at all times.

Working with the Team to analyse target breaches and report issues to management.

Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies.

Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.

Co-ordinate and provide administrative support to the relevant teams to ensure that all the relevant paperwork and clinical information is available.

Attend relevant meetings and complete actions regarding the care of patients within the designated pathway.

Identify and suggest improvements that can be made to the patient care pathway and/or the patient pathway tracking process.

Monitor patients on their pathway and proactively find resolutions to improve the speed of treatment by
