Supporting our service users from point of entry into the service and through their recovery journey, helping with social re-integration and enabling them to lead purposeful and meaningful lives.

Due to our continued commitment in supporting people suffering with substance misuse problems,
**Change Grow Live** are looking for a
**Recovery Coordinator** to join our
**Birmingham **service, working specifically with prison leavers to improve outcomes and continuity of care rates on release. This is an excellent opportunity for someone with experience of substance misuse issues, looking to take the next step in their career with a charity that will support your continued personal and professional development.

**Hours**:37.5hrs per week

**Salary**: £26,393 - £30,316 per annum dependent on experience

**Responsibilities**:
**About the role**:

- Work alongside the Entry Into Service team with a focus on managing all prison leaver referrals for the relevant hub in the community
- Complete and log accurately all relevant assessments and interventions such as triages, alcohol audits, SADQs, recovery assessments, criminal justice assessments, risk assessments, DBST, harm reduction and others
- Support with NMP medical appointments, nurse reviews and psychosocial support as and when required
- Promote engagement with external professionals and agencies, with a service user led focus around community involvement
- Work and liaise with other agencies involved with the service user’s broader care plan including hospitals, GPs, community mental health teams, probation, police, prisons, housing and pharmacies
- Carry out key harm reduction strategies such as offering Needle Exchanges, distribution of Naloxone, both after completion of appropriate training or demonstration of relevant experience
- Support the community hubs open access activities as and when required

**About you**:

- Experience in managing caseloads of people ideally within a CQC registered service or similar
- An understanding of drug and alcohol issues and experience of working within a related field
- An understanding of the criminal justice system and experience of working within a related field
- A commitment to multi-agency working and establishing partnerships with other professions
- Accomplished written and verbal communication skills and a high degree of personal IT competency
- The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information e.g. case management reports and high-risk referrals
- Excellent time management skills and an ability to work on own initiative, prioritising accordingly
- Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering the wider service and specific team objectives
- Good working knowledge of mental health interventions, services, and good practice
