Health and Social care teams across Wokingham, along with local voluntary and community organisations and other colleagues, are developing a more co-ordinated approach to the care and support we offer our local residents. This approach will mean that local communities and service providers work more closely together at a neighbourhood level, based around groups of GP surgeries (clusters), to ensure services are more responsive to local needs and improve people's experience of care and outcomes. The aims of this way of working are that:o People's care and support is more coordinated and provided more quicklyo Communication is improved between staff and with people who use their serviceso People find it easier to access help when they need ito High quality support is provided to people to meet their needso Information and support resources across the Neighbourhood Cluster meet and, where possible, exceed patient and service user requirements and expectations.
