JOB TITLE **Care Coordinator**

LINE MANAGER **Home Manager**

MAIN PURPOSE **To support the Home Manager in delivering a professional, quality assured service. Taking responsibility for care delivery, organising staff team to ensure individual Residents needs are met.**

REQUIREMENTS **To set a good example to others and adhere to the Social Care**
**Codes of Conduct.**
- To hold QCF minimum level 3 and be willing to study for level 4.
- Be a good communicator able to disseminate information clearly and concisely to all levels of staff.
- Understand the company aims and objectives and have understanding of others roles in organisation
- To demonstrate knowledge and understanding of residents conditions, abilities, needs and wishes
- To have involvement in care planning and risk management process
- To demonstrate commitment to equality and diversity, And adhere to

Company policy
- To take a lead role in promoting, organising and updating the key worker system in the home.

MAIN DUTIES AND **Able to organise and guide care team to deliver 24 hour continuous**

R care. Including producing weekly care rotas to maintaining safe
ESPONSIBILITIES staffing levels each shift, reflecting needs of residents and planned
events.
- To continuously assess and monitor Residents for changes in dependency acting in their best interests.
- To actively implement & evaluate Service Users care plans, risk assessment and other monitoring tools.
- To promote health and safety practices within the home reporting any issues, problems or accidents immediately maintaining confidentiality
- To establish and maintain good relationships between Service Users, relatives and staff.
- To participate in the homes social and activity programme helping to plan and implement.
- To ensure safe administration of medications following the Company

Policy and Health Authority guidelines.
- To adhere to the Company’s policies and procedures and to ensure others are fully conversant with them.
- To allocate duties and tasks to Care staff on a daily shift basis
- To evaluate services delivered daily to maintain high standards

- food and drink, personal cares, managing continence, safe supervision of residents and assist the housekeepers to maintain high level of cleanliness of service users personal and communal areas.
- To continually improve and update own professional knowledge.
- To assist with induction and regular training of all care staff.
- Be involved in staff personal development programme, carrying out supervisions and appraisals for staff.
- To carry out other duties requested from time to time, which is deemed reasonable and appropriate to knowledge and skills.
- Report any concerns about a service user or the safe running of the service to the Home Manager.

TRAINING **You will be required to attend all mandatory and statutory training including: a) Health & Safety b) Fire safety and awareness c) Safe-guarding vulnerable adults d) Infection control e) Handling and Moving f) COSHH g) Medication h) Person Centred Care i) Care Planning and Record keeping**

SUPERVISION & **You will be required to participate in developing a personal**

P development plan twice a year.
ERSONAL

DEVELOPMENT PLANS **You will also be required to participate in supervision sessions every two months.**
