Referral and Pathway Coordinator

To provide an efficient and responsive administration service, directly supporting the Referrals and Service Improvement Lead and wider clinical services teams. To act as an initial point of contact for families and professionals, responding in a sensitive and professional manner. To support with the referrals process from initial enquiry and receipt of referral through referral panel to decision and advising the family and referrer of the outcome of their referral. To support the work of the Service Development Department to proactively ensure Demelzas services meet the evolving needs of families.
To adhere to safeguarding policies and procedures, escalating any concerns as appropriate. KEY RESPONSIBILITIES Referrals Providing administrative support to the Referrals and Service Improvement Lead and wider clinical services teams to coordinate a smooth-running referral process. To work with allocated teams to maintain a caseload management system of referrals. Requesting and collating medical and social information from professionals such as GPs, Social Workers and hospital teams, and updating patient records.
Support with cover for the Referrals telephone enquiry number, ensuring a professional response in a timely way. Funded/commissioned placements To support with the monthly funded placement invoicing. Communicating with commissioners regarding funded placements and contract arrangements. To ensure commissioners of funded/commissioned placements are kept up to date regarding any relevant information relating to the placement.
Administration Assisting designated Service Lead(s) with collation of data and presenting in the most appropriate format, such as PowerPoint, graphs, written reports etc. Using the Care Database to produce reports to provide services with relevant data regarding their services and service users, and supporting monitoring of Key Performance Indicators. Providing full administrative support to any meetings relevant to assigned team(s), including organising meetings, inviting attendees, preparing documentation, taking and distributing minutes. This will include but not be limited to referral panel, team meetings, meetings with external professionals, review meetings and MDT.
Assisting assigned team(s) to maintain good communication internally and with other departments. To be a local Care Database administrator, effectively participating in meetings and disseminating relevant updates to assigned team(s). Receipt and actioning of post. Coordination and ongoing management of effective filing and archiving systems in accordance with data protection legislation.
Offering organisational support as required and responding to requests from other departments for data in a timely manner. To work as a team, supporting other team members as required and covering during absence of a colleague within the Service Development Department. To provide local support for health & safety tasks, including but not limited to risk assessments for events and activities. Volunteers Induction, direction and continued supervision of admin volunteers working within the assigned team(s).
Governance To ensure that data contained in the Care Database records of children and young people and family members is entered and updated accurately, in a timely manner and as changes in circumstances arise. To support the wider team in compliance of data protection legislation at all times, including obtaining and recording appropriate consents from service users. Education Offering basic Care Database training to new starters within the Directorate. To support in inductions for new team members as required.
Actively supporting Administration Volunteers in developing their skills and ensuring continuity of tasks during periods of absence.
