Advanced Complex Care Practitioner

An exciting opportunity has arisen for a dynamic individual to join the Whitby Community Team as an Advanced Complex Care Practitioner, working in a senior nursing role to support the community team and a varied caseload of patients. You will need to be hard-working, passionate, driven, with a problem-solving approach to patient and team management. You will work in an integrated multidisciplinary team, working closely with social care, GPs and AHP colleagues to deliver individualised, safe and effective, high-quality care.

The post holder will proactively identify, case manage, and provide appropriate intervention for adults and older people who have varying health needs, including frailty, complex and long-term conditions. With the overall aim of supporting patients to remain at home or in their normal place of residence, avoiding unnecessary hospital admissions. Also facilitating early discharge where patients have been hospitalised.

You will be the Senior Nurse for complex care and frailty for people in their own homes within the Whitby Community Team. You will be a coordinator between GPs, social care, acute hospitals, discharge planning and AHP services, and will take on responsibility of the team when the Clinical Lead Band 7 is on Annual Leave. A consideration would be made for a Band 6 developmental post.
