Please see the full job description for more detail in supporting information JOB PURPOSE: To work as a member of the Community Health Care Team providing phlebotomy services to housebound patients in the community KEY TASKS: To visit patients in their own homes, carry out phlebotomy tasks taking into account good practice for infection control, manual handling and positioning, return bloods to the health centre for transportation to the pathology lab. Maintain accurate records of interventions and feedback to the nursing team any observations made during visits. Communication and Team Working Communicate effectively and sensitively with patients, relatives and colleagues, referring to senior staff as necessary. Answer the telephone and relay messages accurately, courteously and competently.

Remember you are a visitor into patients homes and must be professional, friendly and helpful. Work effectively as a member of the community healthcare team, offering support to colleagues and requesting assistance as necessary. Record care given accurately and concisely via SystmOne and summarise in notes in the patients homes. Privacy, Dignity, Equality Act to promote patients equality, diversity and rights at all times.

Ensure provision of privacy and dignity in all activities. Ensure all care is person centred and delivered with the informed consent of the patient. Act to promote active participation for patients under the care of the Trust. Maintain patient confidentiality.

Infection control Adhere to the principles of infection control demonstrating effective hand hygiene and awareness of potential cross infection. Undertake basic cleaning and decontamination of own and communal equipment between patient use and/or storage. Administrative Enter data into computerised systems. Report repairs/maintenance issues appropriately.

Identify stock and take appropriate action. Participate in teams meetings and keep up to date with team communication. Personal Development Demonstrate insight into own personal development needs and access development through an agreed Personal Development Plan. Attend mandatory training, and complete other in-service training (including e-learning) as necessary.

Maintain and develop standards of practice through continuous personal development (CPD) in line with developments and service delivery requirements. Complete the initial training period required to demonstrate knowledge, skills and attitudes in line with Care Certificate Standards. Complete organisational competence assessment requirements for the job role and clinical tasks.
