The Head of Patient Safety learning is a new post within one of the largest acute care provider organisations in the NHS. This role is pivotal in optimising the organisations learning from from harm (Safety I) and excellence (safety II) agendas, aligned to the Patient Safety Incident Response Framework.

To be accountable to the Director of Patient Safety for all aspects of the Trusts patient safety learning & improvement approach and associated work plans.

The post holder will be expected to lead on core aspects of patient safety learning, ensuring that the trust meets its obligations as set out within The National Patient Safety Strategy, Regulatory Compliance and evidence based practice.

The Head of Patient Safety learning will occasionally be required to deputise for the Director of Patient Safety and /or Deputy Director of Patient Safety & Deterioration at both internal and external meetings/events.

Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.

The merger provides an opportunity to reconfigure services in a way that provides the best healthcare services to the city and improves the quality of care and health outcomes that patients experience.

The Trust runs Aintree University Hospital, Broadgreen Hospital, Liverpool University Dental Hospital and the Royal Liverpool University Hospital.

It serves a core population of around 630,000 people across Merseyside as well as providing a range of highly specialist services to a catchment area of more than two million people in the North West region and beyond.

The post holder will ensure that all staff understand their responsibilities in relation to safe patient care, and will act as an expert resource to the Director of Patient Safety in all areas of patient safety learning & Improvement.

They will oversee the approach to optimising learning from patient safety incidents both internally and externally, ensuring robust systems are in place and maintained based upon clear evidence, and compliant with externally set regulatory frameworks.

To oversee the measurement of learning from patient safety incidents and associated improvement projects aligned to learning from internal and externally identified patient safety incidents. Working closely with the Trusts Clinical Governance team to produce reports both describing and deconstructing Trust wide patient safety incidents and their associated learning.

Developing systems and process to enable a positive reporting culture thatdemonstrates increases in incident reporting across all sites.

Work closely with the Quality Governance and Quality Improvement teams;supporting both the Quality assurance and Quality improvement approach across the Trust.

To work closely with the Director of Quality improvement to ensure that lessons learnt from patient safety incidents are optimised through quality measurement to enhance patient and staff safety across all trust sites and specialities.

To work with the Director of Patient Safety in the on-going development and management of a Trust Quality and Safety First Strategy that reflects the national and local strategic framework for the NHS, particularly in relation to learning from and reducing future patient harm.

To provide analysis/expertise and consultation with existing experts in Patient safety and Human Factors that exist in the Trust to derive sustainable evidence based solutions.

To support the development of Patient Safety learning as an integrated Divisional approach, and to promote closer working with clinical and non-clinical colleagues to ensure delivery of a proactive client centred, safety learning service.

To support the development and reporting against associated KPIs, and further develop service standards for the assessment of patient safety learning within the Trust.

To support an open and psychologically safe approach to learning from incidents, exercising good people management practices and support cultural and behavioural change to develop safer practice.

To ensure that a positive and transparent approach to patient safety is engendered throughout the Trust and that effective frameworks for triangulation are maintained and developed, including supporting patient safety related committees

To support the development, negotiation and maintenance of Trust policies and procedures relating to patient care, safe systems and processes, safe working and a safe working environment, ensuring that they support the Trust strategic aims and reflect current best practice.
