Job Purpose The post holder will primarily be responsible for the triage process of all referrals to the Neighbourhood Model. In addition, they will provide senior clinical advice and support to patients/carers and for staff working in the Neighbourhood Teams and at times work clinically within the services of the model. In doing so, the triage clinicians assess, plan, deliver and evaluate patient centred care to a specialist level ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.

Key Responsibilities 1. Clinical 1.1 Be responsible for managing referrals into the Neighbourhood Model, demonstrating sound clinical decision making and the use of Electronic Patient Record (EPR) systems including SystmOne and Leeds Care Record providing clear evidence of effective management. 1.2 Participate in the Hub rota to manage the workload, effectively triaging planned and unplanned work according to staffing capacity, safety and competence. 1.3 Manage the safe transfer of care between care providers and make appropriate referrals to other care providers outside of the Neighbourhood Model.

1.4 Make consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensure clinical reasoning is reflected accurately in case records. 1.5 Holistically assess/reassess and continuously review allocated patients to ensure their clinical needs are being met, collaborating with the multidisciplinary team and other providers to ensure all identified needs are addressed. 1.6 Demonstrate a high level of clinical competence and a high level of specialist knowledge with regard to care delivery, thereby demonstrating clinical credibility and being a credible role model within the team. 1.7 Spending time in clinical settings within the Neighbourhood Model to maintain a clear understanding of the different care pathways in a variety of settings.

frequency. 1.8 Providing oversight and support to the clinicians and patients/carers as required to ensure safe, effective, quality care is delivered to all patients in the Neighbourhood Model. 1.9 Electronically plan patient visits using SystmOne and e-Community for patient care ensuring integrated care is promoted. 1.10 Ensure patients requiring more in depth case management from a multidisciplinary perspective are referred into the Case Management Meeting process within the Neighbourhood Team.

Ensure team representation at Case Management meetings. 1.11 Maintain and deliver compassionate patient focused care in accordance with LCH guidelines, policies, values and behaviours. 1.12 Ensure that documentation standards, whether written or electronic, are maintained in accordance with Trust guidelines and regulatory bodies. 1.13 Comply with the organisation’s Infection Prevention and Control requirements, including “bare below the elbows” dress code when delivering direct care to patients.

1.14 Communicate with the patient and/or their family/carer to ensure they are involved in agreeing goals regarding their identified needs and have access to appropriate information and knowledge, which may be complex, to inform decision making. 1.15 Promote evidence-based practice and use a health coaching approach wherever possible with patients so that they are empowered and supported to manage their own care. 1.16 Balance clinical risk and clinical decision making against context, ensuring patients receive safe, effective, and timely care. 1.17 Manage the safe transfer of care between care providers and make appropriate referrals to other care providers within and outside of the Neighbourhood Model.

1.18 Demonstrate good insight, knowledge of and adherence to Trust Safeguarding policies and procedures to safeguard the health and wellbeing of children and vulnerable adults. 1.19 Recognise ethical and legal issues which have implication for nursing practice and take appropriate action in line with NMC guidelines. 1.20 Maintain a safe working environment, ensuring the safe use and efficient management of equipment and supplies. 1.21 Work in accordance to the clinicians regulatory bodies code and fulfil the requirements of revalidation or re-registration.
