A vacancy has arisen for a full time Discharge Facilitator (37.5hours per week)

The Discharge Facilitator will need an interest in, and a passion for improving safe and timely hospital discharge for patients. You will gain invaluable experience in this important aspect of inpatient care.

As the discharge facilitator, you will support ward staff to discharge patients. Utilising the Discharge Lounge, chasing up TTOs or any outstanding actions, liaising with families/carers, calling Care Providers to re-start care, booking transport, chasing for updates on progress, liaising with support services in the Community (i.e. voluntary sector), etc.
