An exciting opportunity has arisen for 3 hardworking, caring and compassionate individual's to work as part of the Haematology Clinical Nurse Specialist Team for Swansea Bay University Health Board.

The CNS coordinator will work effectively within a multidisciplinary team and across other agencies working closely with the Clinical Nurse Specialist to support haematology patients throughout their journey. The CNS coordinator will demonstrate a level of skill and understanding which enables them to undertake specific tasks delegated by the CNS or member of the MDT and in accordance with relevant procedures, guidelines and policies. They will undertake both routine and specifically identified tasks which they have been trained and assessed as competent.

It will involve supporting aspects of care delivery, patient pathway co-ordination, assessing the needs of patients and carers and providing information, help and support to those living with a haematological diagnosis

As an extended member of the haematology MDT, the role will support effective communication between patients, their families and carers, primary and secondary care and with third sector agencies.

The Band 4 coordinator will be expected to support the clinical nurse specialist in managing the needs of patients diagnosed with a haematology diagnosis.

The post holders require a willingness to be flexible in order to meet the needs of the service.

NVQ level 3 is essential with the willingness to work towards level 4.

You must have excellent communication/interpersonal skills and the ability to work on your own initiative as well as part of a team.
