**Salary: £20,720 per year, Pro Rata**

- IMPACT Provider Collaborative are funding ‘Assertive Transitions Service’ (ATS) to expedite the safe discharge of forensic mental health hospital patients into the East Midlands.As part of this team, Peer Support Workers will work with patients on thewards, through the transition process and then support them in community living which will cover Leicestershire and Northamptonshire.
**About the role**:
- The ATS will use Peer Support Workers in supporting the transition for identified, local patients, from secure care to the community.
- You’ll use the knowledge gained through your own lived experience to inspire hope and belief that recovery is possible in others.
- You’ll be working on a ward alongside the clinical team to help patients achieve their recovery goals in preparation for their transition.
- Working with people with complex psychological needs, you will need to be emotionally and mentally resilient.
- You’ll be expected to draw upon your experiences of recovery and will be given training on the most effective way to so this.
- The posts will also involve contributing to the promotion and development of the Peer Support Worker role, particularly within secure mental health services.
- You will receive induction, training and on-going supervision and an opportunity to spend time on a ward as well as regularly network with other Peer Support Workers.
**What you’ll do**:
- Building supportive and respectful relationships with patients and a range of mental health professionals on the ward and then in the community
- Supporting others using the personal experience and confidence you have gained having overcome similar challenges
- Assisting clinical and non-clinical staff to help people identify their own recovery goals in preparation for transition
- Inputting into ward rounds and writing up patient notes
- Work within a multi-disciplinary team to fulfil patients identified goals for their individual pathway out of secure care
- To link with and support family and friends of patients and involve them all aspects of the transition process and a patient’s pathway
- To accompany patients to appointments/meetings and activities and begin to familiarise them with aspects of life in the community
- To continue work with patients as they transition into the community, identifying potential support services, resources, activities and opportunities and putting safeguards in place
- To champion and promote the peer support worker programme and role within the Charity
- Modelling personal responsibility, self-awareness, self-belief, self-advocacy and hopefulness
**What you’ll need**:
- A background of personally recovering from mental health issues
- Past and lived experience of using secure mental health services
- Resilience and to be able to know when to ask for help or advocate for patients
- A good understanding of safeguarding and identification of risk
- An understanding of factors which can affect recovery in mental health
- IT literacy including Microsoft Office and Internet or a willingness to learn
- Strong verbal and written communication skills
- The ability to work well in a team
- Empathy, good listening skills, approach-ability and common sense
- To work flexibly including some evening and weekend work
- To be mobile, whether using public transport, or as a car driver and owner
- Local knowledge of support services for those in recovery from mental health issues and links with community mental health support
