**Key Role of the Job**

The Specialist Practitioner will play a leadership role within the CMHT to support the delivery of a clinically safe and effective service.
Reporting to the Team Manager / Deputy Team Manager the post-holder will hold a complex caseload and take an active role in ensuring that effective systems are in place for Caseload Management and Clinical Supervision across the team.
The post-holder will take a lead role for agreed areas of work as required.
They will act as expert practitioner to their colleagues offering support and advice where needed, promoting the principles of recovery and service user involvement and support the Team Manager / Deputy Team Manager in ensuring that team members work in
accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding.

**CLINICAL**
To act as an expert practitioner within the community mental health team offering support and guidance to team colleagues.
To hold a complex caseload of service users with severe and enduring mental illness and effectively co-ordinate their care.
To undertake initial assessments where this is clinically appropriate.
To formulate care plans in line with identified need.
To provide recovery focussed therapeutic interventions in line with the agreed care plan and risk assessment.
To assess risk, develop risk management plans, review and adjust in line with the service user's needs.
To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
To use tools which promote user empowerment e.g. Wellness Recovery Action Plans (WRAP)
To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the post holder provides those interventions. This will include physical health monitoring.
To have a proactive approach to the role and needs of carers / families.
To be proactive in monitoring mental health needs and respond in a timely fashion if needs change adjusting the care plan accordingly.
To work with relevant agencies, procedures and legislative requirements, in particular, The NHS and Community Care Act, Mental Health Act, Data
Protection Act, Adult & Child Protection Procedures.
To work in accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding
To promote and deliver evidence based interventions in accordance with those agreed for use within the service

**CLINICAL LEADERSHIP**

To take a leadership role within the Community Team acting as a positive role model and providing expert support and advice to practitioners to support them in the discharge of their responsibilities.
To lead on agreed areas of work for the team.
To ensure that care planning reflects agreed clinical care pathways.
To provide clinical supervision, caseload management, appraisal and development to an identified group of practitioners within the team.
To undertake investigations in line with relevant organisational policy e.g. complaints, critical incident reviews and HR investigations.
To alert the Team Manager / Deputy Team Manager in a timely manner to any concerns about the teams effectiveness or that of any of its members.
To be involved in recruitment and selection.
To work within agreed resources.
