You will deliver high quality care in accordance with guidance given by practitioners/registered staff & should have an SVQ III in healthcare as a minimum/academic equivalent/ be working towards or have the relevant experience in a Health care assistant role.

You must be driven to provide a person-centred care experience for individuals & have a thorough knowledge of frailty. You should have good communication and interpersonal skills as well as previous knowledge, skills and caring experience in managing the acutely unwell adult. You will work flexibly with both health & social care services to enable patients to remain in their own homes whilst achieving the best outcome for patients under the remit of the Hospital at Home Team.

This is a fixed-term/secondment opportunity for 12 months.

Full driving license and car owner is essential.

**Hours**: 37.5 per week.
