Would you like to be involved in finding care for citizens in Wrexham?

An opportunity has arisen to join a busy and friendly team within the Social Services department for 9 months to cover maternity leave. The team covers a range of support services - domiciliary care, residential care, supported living, day care and other services from third sector providers. You will be a point of contact for Providers, Social Work and Finance teams.

Your duties will include accurate and timely processing of care plan information and liasing with a range of providers and professionals to source the care needed - this care could be in someone’s own home or helping them to find a care home to meet their needs.

You will be need to be able to use initiative to collect information from a range of sources.

Strong numerical, Word and Excel skills are essential for this role as the team uses a range of spreadsheets and online databases.

The team are adopting a hybrid working model of working in an office (Crown Buildings) and working at home. You will need to be comfortable using technology to be able to work from any location and to maintain contact with colleagues.
