St Neots PCN’s Personalised Care team consists of a dedicated team of Social Prescribing Link Workers, a Health and Wellbeing Coach and a Care Coordinator. They are a passionate team who deliver personalised care to a high standard. The post holder will be responsible for supporting the team in their day to day work, managing rotas, undertaking case load reviews, identifying training and development needs and being a general first point of contact for any questions, queries and ideas. In addition to line managing the team, the post holder will also hold a caseload of patients in their area of expertise as Social Prescribing Link Worker, Health & Wellbeing Coach or Care Coordinator.

**What you will do**
- Supervision and daily line management of Personalised Care Team members; being a first point of contact.
- Developing induction plans for new members of the team; identifying training requirements and opportunities for the existing team.
- Provide support and guidance through facilitating regular peer review sessions.
- Working with practices to establish and maintain appropriate referral processes.
- Supporting patients through the personalised care model, focusing on strengths and needs of individuals.

**Essential Criteria**
- NVQ Level 3, or Advanced Level or Equivalent experience in health and social care.
- Experience and knowledge of providing support to vulnerable people through lifestyle interventions or motivational coaching through a recognised Personalised Care role.
- Excellent interpersonal skills with the ability to influence change and process.
- Good ability to use IT functions such as MS Office; good knowledge and experience in reporting in SystmOne.
