**Job Summary**
**Care Co-Coordinator**
Full Time
- 37.5 Hours per week.
Monday-Friday 9am-5pm.

**Responsibilities**:
- To work with and support the care coordinating team in organising and coordinating the running of the services we provide, i.e. care plans, work schedules and rosters
- To contribute to writing, monitoring and reviewing of all service policy and practice guidelines.
- To ensure excellent quality of care is provided, carry out regular reviews and evaluations.
- To support the care manager and prepare for inspections and visits.
- To ensure all recommendations and instructions are carried out as requested.
- To maintain records and compliance in line with GDPR.
- To ensure person center plans are in place and reviewed regularly.
**Required Education, Skills and Qualifications**
**Essential for role**
- Must have or be working towards SVQ 4 in social care
- Experience in co-coordinating staff and rotas
- Communication / interpersonal skills
- Good verbal and written skills in English
- Commitment to personal development
- Team player
- ability to build and maintain professional and productive relationships
- Full Valid UK driving license with access to own vehicle
- Experience with all Microsoft office suite, i.e. Microsoft Word, Microsoft Outlook
**Desirable for role**
- Health & Safety / First Aid / Food Hygiene / Moving & Handling Training Certified
- Knowledge in effective service delivery
- Experience of working in a supervisory capacity and conducting meaningful supervisions would be desirable
- Ability to work on own initiative
- Appropriate registration with the SSSC
- Experience in working with call-monitoring systems
- Vision for growth and expansion
