**Job Summary**

An opportunity has arisen for a reliable, enthusiastic, and motivated individual to join our Practice Reception / Administration Team. We are a friendly & highly motivated practice, we are dedicated to patient care and a positive patient experience, caring for 10500 patients.

We are looking for a Care Navigator to undertake a variety of administrative duties to assist in the smooth running of the practice and provide the highest achievable levels of service and including the provision of triaging appointments effectively, supporting the clinicians and other members of the practice team.

We are innovative in the use of IT, we use TPP (SystmOne) and AccuRX.

**The hours of work are 07:30am - 12:30pm**

**Main duties of the Job**

**Key Responsibilities**:

- Triaging appointments via AskmyGP to ensure these are directed to the correct member of the team.
- Initiating contact with and responding to requests from patients via AskmyGP, other team members and external health providers.
- Navigating the appointment system to ensure all appointments are booked into the appropriate slots on a daily basis.
- Receive, assist, and direct patients (Care Navigation) in accessing the appropriate service or healthcare profession in a courteous, efficient, and effective way.
- Responding to the needs of both doctors and nurses during surgery times, giving support with general tasks as requested.
- To be efficient in the use of SystmOne, AskmyGP and AccuRX.
- Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures.
- Filing/scanning/read coding.
- Filing normal blood test results and informing the patient.
- When required, supporting the Medical Secretary with referral forms that require resubmitting.
- Liaising with other NHS services on the doctors and nurse's behalf.

**Person Specification**

**Knowledge and Skills**:
**Essential**
- Good interpersonal and communication skills.
- Ability to work under pressure and prioritise workload.

**Desirable**
- Experience of working in a health care or customer service centred environment.
