Are you looking for a new opportunity to work within a dynamic and forward-thinking community dietetic service? We are looking for a dietitian to join us, working with Primary Care Networks (PCN) in South Stoke Central. You will be responsible for helping establish the adult dietetic services in this PCN. This advanced role is varied, requiring a significant level of autonomy and clinical decision making.

As an employee of MPFT, working as part of the PCN team, you will form part of the well established wider Community Dietetic team including PCN Dietitians and Allied Health Professional (AHP) Teams within MPFT. As part of the service you will have access to regular supervision and appraisal and a wide network of support from our AHP professional leadership team, Dietitians and practitioners across the organisation.

This post will be a combination of face to face and remote working, covering practices within South Stoke Central. It is essential that you are able to travel within the area covered by the PCN team.

**You will**:
provide quality driven, evidence based dietetic interventions and programmes in the primary care network.

Be competent as appropriate to the requirements of the role.

Demonstrate at all times the Partnership Trusts Values and Behaviours

Be an Advanced Dietitian and have a detailed, broad professional and clinical knowledge. The post holder will work across a broad spectrum of clinical conditions as guided by the needs of the Primary Care Network, this is likely to include Diabetes, obesity and nutritional support..

Be responsible for a defined population of service users in a variety of settings, including the patients home and clinics across the Primary Care Network locality.

Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages for patients and families

Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.

Demonstrate initiative and be creative in finding solutions to problems.

To undertake comprehensive/advanced assessments of patients, including those with complex needs, using investigative and analytical skills to formulate individual management and treatment plans, using clinical reasoning, and utilising a wide range of treatment options to formulate a specialised programme of care.

To take a lead in specialist area/complex caseload and providing advice and support to others. The specialist care/support plans may include the following:

- Promoting independence/self-management and maintaining safety
- Increased functional ability
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance
- Case management/ treatment /care plans
- Palliative and end of life care

To use expert knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service user’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

To maintain and develop skills and knowledge to deliver a range of interventions and sets an example of good practice to other team members including skill sharing.

To assess, diagnose and manage complex/specialist dietary complications through the analysis of patients' biochemistry, anthropometry and diet history and provide specialist dietetic interventions underpinned by dietetic theory and evidence.

To develop and independently run dietetic therapy groups, providing evidence based outcomes following assessment and intervention

Provides teaching/training in specialist area to the wider workforce.

To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy, in particular when imparting potentially distressing information. This may be in regard to the nature/prognosis of their condition and implications of the same including managing any conflict that may arise.
