**Job Title**: Support Worker
**Responsible to**: Directly accountable to and taking direction from the employer and the employer’s representative (Mum)
**Hours**: 15 hours per week & additional hours to cover annual leave and sickness of other PA’s
**Hourly Rate of Pay: £13.00 per hour**
**Ba**s**ed at**: Employer’s own home in Stoke on Trent and out in the local community.
**Job Purpose**
To work one to one with an adult with Autism and learning disability. The young gentleman has a good sense of humour and lives in his family home with his parents and sibling. The purpose is to build a trusting professional relationship with the young gentleman. To support him in developing confidence in social settings and to develop independence, promoting a fulfilling quality of life. The young gentleman enjoys good food, going out shopping, going for cake & coffee, cinema, bowling and day trips out.
He also has a real passion for all things acting and film related.
**Main duties**
- Support in ensuring gentleman is appropriately dressed i.e helping with shoelaces, decisions on appropriate clothing choice.
- To support young gentleman to access places of interest
- Transport gentleman to and from home to places of interest
- Helping gentleman make independent choices
- Support with shopping
- Ensure gentleman safety in public spaces i.e road crossing
- Support and attend health care appointments
- Report any issues to gentleman’s parents
- To perform such other duties as may reasonably be required

**Special Conditions**
The post carries a 3 month’s probationary period.
