Children and Young People’s Patient Ambassador

Hamara HLC are currently recruiting a Patient Ambassador to work in collaboration with General Practice services to patient, many of whom often find it difficult to engage with healthcare and certainly find it difficult to navigate the healthcare system. The Patient Ambassador aims to provide a mixture of reactive and pro-active communication to improve the healthcare experience of all patients to support their ability to ‘choose well’ and link up with appropriate services available.
The Patient Ambassador provides an ‘in practice’ presence which can undertake more detailed and person centred communication than a clinician is sometimes able to complete and aims to educate and motivate the patient and build links with existing community and statutory bodies and in particular connect for Health.
We have identified particular needs across our large population of young people and those who have English as a Second language and this role aims to develop systems and processes which help us to improve how we interact with these patient groups.

**Purpose of the role**
The main purpose of the Children and Young Peoples Patient Ambassador is to provide personalised support to individuals, their families, and carers to take control of their wellbeing and improve their health outcomes.
Young people from similar backgrounds with equivalent opportunities can end up having very different experiences of life. Some feel isolated in a place where life has lost meaning, while others find a life of meaning and purposeful relationships. In general, that is. Reality is more fluid with young people fluctuating daily, weekly, and annually between thriving, struggling and something in between.
Diverse activities and services are available in our communities, but those who may benefit from them most don’t always feel able to take part. What they need is a helping hand, someone to listen to their needs and connect them, even take them personally to a nearby offer. Our Children and Young Peoples Patient Ambassador will help young people make the connections they feel unable to make by themselves to respond to their unmet needs. The connection could be to an advice or counselling service, a medical or health solution or to something sporty, arty or social. Or to volunteering, or literacy support. Or a combination of several of those things - all of which will give them a strong foundation, positive wellbeing, basic skills, and reliable relationships.

**Key tasks and responsibilities**
**Provide personalised support**:
v Using motivational interviewing and other techniques, provide personalised information, advice and support to children and young people and their families, signpost or refer (with consent) them to appropriate activities, services and support which will help meet their needs, circumstances, and preferences. Meet children and young people on a one to one basis and give them the time they need to help focus on ‘what matters to me’.
v Work alongside those referred (participants) to address the barriers to participation and things which are negatively affecting their wellbeing. This includes addressing the wider determinants of health, including lack of education, poor housing, un/under-employment, physical inactivity, etc.
v Empower participants to maximise the control the have over their lives through enabling them to assess their own abilities, identify goals, take charge of decisions which affect them and improve their ability to self-care. This will involve co-producing action plans and facilitating their follow-through.
v Where appropriate, physically introduce children and young people to community groups, activities and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included and receiving good support.
v Effectively manage and prioritise a caseload of participants, ensuring ambitious performance targets and project objectives are met. This will be done in accordance with the needs, priorities and any urgent support required by children and young people and will include home visits where appropriate.
v Develop the outline plan for the role in collaboration with others and look to deliver improvement in areas identified in the Core 20 PLUS 5 Approach and in services especially important to Network Members including improving the uptake of childhood immunisations within the PCN area.
