We have an opportunity for a part time (22.5 hours) therapy assistant practitioner to work as a key member of our Reablement Team. This is a multidisciplinary team comprising Occupational Therapy, Physiotherapy, Speech and Language Therapy and Therapy Practitioner Assistant. The team work together to provide goal orientated neurological rehabilitation to maximise patients’ independence following discharge from hospital. The team is based at Nottingham City Hospital.

You must have a passion for delivering patient centred care, supporting the delivery of occupational therapy, physiotherapy and speech therapy rehabilitation interventions. The post will involve travel to patients’ homes across Nottinghamshire to provide rehabilitation interventions. A full driving licence and access to a vehicle for work are therefore essential to role. You will also need to be computer literate, with a willingness to learn how to use of a range of hospital ICT systems.

There is an expectation that you will be actively involved in project work & engage in ongoing service developments, working closely with community colleagues around the ongoing rehabilitation of patients and supporting safe and effective transfer of care.

You will work alongside the multidisciplinary team in the assessment, intervention and delivery of care for patients, as well as delegated administrative duties.

Our HCPC registered AHP professionals comprise of physiotherapists, dietitians, occupational therapists, orthotists, prosthetists and speech and language therapists. We have a Chief AHP to lead us on our strategic direction, and many of the pathway leadership team have pathway AHP backgrounds to support our staff. There are also Practice Development Therapists to support career development for all.

The Therapy Assistant Practitioner (TAP) provides appropriate assessments, treatment and discharge planning for patients under the supervision of a senior therapist.

The post holder will be required to work closely with the multidisciplinary team to develop and implement rehabilitation programmes, as well as facilitate discharges with the support of senior staff.

To use effective communication skills to gain co-operation with treatment where there may be significant barriers to understanding or ability to co-operate with the treatment.

To discuss caseload management with other clinicians and provide advice and support on a day to day basis to other support workers.

To refer onto appropriate hospital and community services.

Essential criteria
- GCSE (A-C) English/Maths or equivalent
- NVQ/QCF level 3 in healthcare or equivalent relevant qualification
