We have an exciting opportunity to recruit a Band 6 Mental Health Practitioner who has an interest in Adult and Older Persons Mental Health to the Newcastle North PCN through Additional Roles Reimbursement Scheme (ARRS). The post will be integrated into the PCN mental health and multi-disciplinary team working across 5 member Practices.

The adoption of this role is an exciting and new development that will build on and complement the progress of the new model of primary mental health care support.

Newcastle North PCN comprises of 5 separate member GP Practices in 4 locations working together to provide patient care to a population of just over 35,000.

They are an enthusiastic, dynamic and friendly PCN who constantly strive to improve patient pathways and health care outcomes.

We are seeking to find a Mental Health Practitioner to join the team and make a difference, providing high quality assessments and short-term interventions to support this patient group.

The role delivers timely access to an integrated, high quality, service user-centred service in primary care that enables people with mental health needs and/or social care problems (regardless of diagnosis) to improve their well-being and functionality, thereby preventing, reducing or delaying the need for more specialist services (either for mental health, physical health or social care needs).

The Mental Health Practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team, as well as the Community Mental Health Multi-Disciplinary Team (MDT) with ready access to a wide range of support and expertise across both.

The focus of practitioners’ work within the primary care setting will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services. The practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway, as well as provide a range of evidence based, recovery focused interventions and support to service users and their carers where appropriate.

The post holder will demonstrate outstanding autonomous and critical thinking and be a caring, compassionate and committed experienced mental health professional.

They will, whilst acting within their professional boundaries, provide care for the presenting patient from initial history taking, assessment and management of care, including escalation to a more senior clinician. They will demonstrate safe, competent clinical decision-making and expert care for patients within the practice and wider primary care network.

They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. They will liaise with other professionals and agencies both statutory and non-statutory and provide a link between primary and secondary services from a North Staffs Combined perspective.

Key Duties/Responsibilities
- To undertake assessments of individual client’s difficulties, establishing risk and determining the appropriate course of action taking into consideration all possible options. The relationship should adopt a ‘partnership approach’ engaging service users in the positive management of their health and wellbeing.
- Proactively identify and manage treatment plans for patients with dementia and serious mental illness.
- Diagnose and manage both acute and chronic conditions, integrating both drug
- and non-drug-based treatment methods into a management plan.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients in order to support compliance with and adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, escalating to senior MHP if necessary.
- Take prompt action on changes in condition, liaising with other caregivers, members of the clinical (multi-disciplinary) team and/or other agencies when necessary, reviewing care and communicating any amendments to the medical records.
- To have responsibility for seeking support and advice from General Practitioner or senior MHP/supervisor in respect of issues and difficulties with individual patient care. To attend regular clinical supervision with the senior MHP in order to address caseload management issues.
