The Mental Health Community Care Assistant role within the Access Team requires SVQ Level 3 in Social Services & Healthcare / Health & Social Care, or equivalent, and experience in mental health recovery related work. The Community Care Assistant will support the Mental Health Practitioners with assessment, personal outcome and recovery focussed care planning and review, and will be directly involved in the delivery of support for adults with mild to moderate mental health needs living in Perth & Kinross. The post holder will participate in mental health triage and support with third sector organisations working shift patterns and one weekend in four, enabling flexibility to the overall service. You will deliver support in line with policy, procedure and best practice.

**The Individual**:
The support offered will be short term and recovery focussed, and will encourage the development of self-management skills to support service users to move on from, or live better with, the symptoms of mental ill health. Support can be delivered on an individual or small group basis. You will be able to develop effective interpersonal relationships with service users, carers and other agencies, and work in partnership with other locality based partners to offer support focussed on personal outcomes and recovery. The post holder will demonstrate good written, IT and verbal communication skills, and demonstrate skills in assessment, care planning and review processes. You must have a genuine interest in working with people in a recovery focussed way to improve their mental health and wellbeing and be able to demonstrate skills in groupwork facilitation.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
