JOB DESCRIPTION/PERSON SPECIFICATION

YOUR APPLICATION

**Job overview**:
**Are you an experienced Mental Health Practitioner looking to work autonomously, in a forward thinking, fast-paced environment?**

Whether you are a Registered Nurse, Occupational Therapist or Social Worker; this is the ideal challenge for someone who is looking to make a difference to the local community of North Norfolk.

You’ll be working alongside an experienced lead mental health practitioner, as well as alongside Mental Health Practitioners based at the different surgeries.

Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.

The North Norfolk (NN3) Team are passionate about developing their mental health provision; they’re proactive and enthusiastic and need a forward-thinking, driven mental health practitioner to help them achieve their aspirations.

NSFT and North Norfolk Primary Care (PCNs) have come together in partnership to embrace integrated mental health care delivery for our local population. The North Norfolk Primary Care Network (NN3) has 4 individual practices; Market Surgery, ‘Drayton, St Faiths & Horsford’, ‘Reepham & Hungate Street’ and Coltishall Medical Practice who, combined, care for 46,000 patients.

If you are excellent at building relationships, a great communicator and feel confident making your own decisions - we would love to hear from you

**Main duties of the job**:
As a PCN MH practitioner, you will work across the interface between Primary Care and Secondary Mental Health Services undertaking the planning and delivery of direct patient care.

We’re committed to training, continuous professional development and ensuring all members of the team can access resources to help them to develop and progress. When you join us, you can expect to be supported from the start; you’ll spend time with our community teams learning about the different roles they have, as well as shadowing other members of the PCN.

These roles, although new have been piloted in our Early Adopter sites for the past year so the workload is defined, manageable and appropriate to allow for patient centred care and other activities

**Working for our organisation**:
Here at NSFT we pride ourselves on being a welcoming, talented, friendly and supportive team who like nothing better than sharing experiences and learning from each other. In addition to ongoing training and development opportunities, we are committed to providing an environment in which you can thrive.

**Why work for us?** We have challenges as a Trust, but we have ambitious aspirations, are pushing ahead with exciting transformation work and we need dedicated individuals to support us on our journey. We have strong, established nursing networks coupled with an exceptional leadership team who will ensure you are truly cared for and cared about.

**Why Norfolk and Suffolk?** The people here are warm and welcoming, you’ll never be far from the beautiful coastline or Broads National Park. We’re an hour and a half away from London and have an international airport in Norwich too. Our villages, towns and cities are packed full of history, independent cafes, shops and theatres. We have excellent shopping, eating out, top ranking schooling and affordable house prices too.

**Detailed job description and main responsibilities**:
Quote from a member of staff
- ‘I enjoy working closely with GP colleagues to facilitate appropriate care for patients and provide a link between the GP surgery and secondary mental health services, knowing that these fields have occasionally felt like very separate and distinct par
