Pastoral Support Coordinator

Join our team as a Pastoral Support Coordinator, where you'll play a crucial role in providing emotional and well-being support to individuals within our care homes community. Working closely with residents and their families, and our multidisciplinary teams, you'll foster a culture of comfort and support, particularly for those in the end-of-life stage. Your responsibilities will include emotional, psychological, spiritual, cultural, physical, and bereavement support, as well as educating and supporting staff in end-of-life care practices.
**This post is working 36.25 hours per week, Monday to Friday with regular travel across our resources and you will require access to a vehicle for work purposes.**
**What you will be doing**:
- Managing a complex caseload, ensuring the safety and emotional well-being of individuals.
- Prioritising support according to individual requirements.
- Addressing discriminatory practices and inappropriate behaviour, promoting respect for all.
- Supporting colleagues in making timely, positive decisions and identifying local resources for enhanced well-being.
- Facilitating multi-agency collaboration to ensure the best support possible.
- Providing feedback to improve service provision based on changing circumstances.
- Recording vital information related to end-of-life care planning.
- Offering ongoing 1-1 support during times of transition.
- Facilitating support groups for relatives and friends as needed.
- Providing immediate emotional support when necessary.
**What you will need**:
- Significant experience managing an emotional well-being caseload: Prior experience in a similar role or in a related field, such as social work, counselling, or healthcare.
- Experience working with adults in social care/emotional well-being contexts: Experience in providing support to adults, particularly in settings such as care homes or healthcare facilities.
- Experience implementing systems and processes: Experience in developing and implementing procedures and protocols ensures that support services are delivered efficiently and effectively.
- Skill in assessing well-being needs sensitively: The ability to assess individuals' emotional, psychological, spiritual, and cultural needs with sensitivity and empathy.
- Ability to assess and enable access to support: Understanding the range of support services available and knowing how to connect individuals with appropriate resources and services.
- Comprehensive note-taking skills
- The ability to support and guide colleagues in providing emotional support and implementing best practices in well-being.
- Critical reflection on social, psychological, emotional factors: Being able to critically analyse and reflect on various factors influencing individuals' emotional well-being allows for more effective support planning and interventions.
- Support in recognizing and challenging discrimination: Addressing discrimination and promoting inclusivity is important for creating a supportive and respectful environment for all individuals.
- The ability to remain calm, composed, and proactive in managing difficult or sensitive situations is essential for providing effective pastoral support.
- Clear and effective communication skills are necessary for building rapport, facilitating discussions, and conveying information to individuals, families, and colleagues.
