An exciting opportunity has arisen to lead and join the patient flow team.
We are looking to recruit an experienced nurse or AHP with a professional registration with the NMC, HCPC or GPC with excellent communication skills.
The main duties of the role include leading the patient flow team which includes the discharge lounge and bed management functions, and to manage the ultilisation of beds across the Trust whilst ensuring a high standard of safe, coordinated patient care is maintained. They will be the first line contact for enquiries and incidents on the hospital sites relating to patient flow and play a key role in troubleshooting these.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. - To participate in and take lead responsibility for the 24 hour site management service that delivers strong clinical and professional leadership, provides professional advice and expertise to clinical and administrative staff and has accountability for safe and efficient patient flow across the Trust
- Responsible and take ownership for own contribution and that of team in delivering the key Trust access targets (i.e Emergency Department targets) and accountable for reporting and escalating exceptions.
- To manage the patient flow team which includes the discharge lounge and bed management functions by providing clinical and professional management and strong leadership.
- To manage the utilisation of beds whilst ensuring a high standard of safe coordinated patient care and consider the impact Trust wide and ensuring that patients are transferred to the best possible location whilst maintaining key performance targets
- Responsible for actively managing demands on the service and resolving conflicts as they arise and liaise closely with the clinical staff ensuring accurate information is available
- Be the first line contact for enquiries and incidents on the hospital sites relating to patient flow and the patient flow team
- To communicate clearly and effectively in collaboration with all members of the multi-disciplinary team to ensure a smooth and safe transfer of patients into and out of hospital
- To play a key role in troubleshooting problems that occur last minute with patient discharges
- To manage and coordinate transport service utilization in conjunction with third party suppliers
- To ensure timely repatriations from other healthcare providers
- To participate in a major Incident Procedure as per Policy
