At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier We’re looking for an Triage and Support Officer to join our One You Lincolnshire service. One You Lincolnshire are a integrated healthy lifestyle service who provide Smoking Cessation, Adult Weight Management, Move More and Drink Less services. One You Lincolnshire also provide a Strength and Balance programme for older adults, and a Child and Family Weight Management programme Gloji Energy.

A typical day as an Triage and Support Officer might include:

- Use a variety of systems and approaches to ensure clients get the best, positive and client-centred approach when supporting them through our onboarding process.
- To offer guidance and support to clients both on the phone and in written format to ensure they are best aligned to the most appropriate health intervention offer for them and their needs. (Training will be provided if successful).
- To contact clients daily who wish to start their journey with us, being a friendly, warm and empathic voice that represents our service positively and to correctly record their information on our bespoke systems accurately and timely.
- To respond to requests for information from clients, partner organisations and the general public.
- Taking responsibility for your own training and development, that way we can make the biggest impact
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments - we’re always looking to share knowledge and resources
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

**Contract**: Permanent, Full Time (37.5 hours)

**Salary**:Starting at £19,000 per year (rises to £21,000)

**Location**: This job is based in Lincoln. You will be expected to work a evening every 2 weeks, there will opportunities for home working.
