Brighton & Hove City Council is looking for a Care Team Lead to join a new service within our 0-24 Specialist Community Disability Service and this is an exciting opportunity to be part of it.

The team will sit within the 0-24 Specialist Community Disability Service (SCDS) but will work closely with teams across education, health, social care and within the community.

**About the role**:
The transforming care lead is a highly visible leadership role that will help shape, develop and implement the new keyworker service across Brighton and Hove. You will lead and inspire a small team of keyworkers to achieve positive person-centred outcomes.

You will be responsible for staff developments and supervision and ensuring that the service continues to evolve to ensure outcomes are met.

The role will operate across education, health and social care systems as well as the voluntary sector, working in collaboration with key stakeholders to share and model good practice and learning, whilst being able to challenge the systems to unblock barriers and achieve positive change.

We are looking for someone who is able to communicate clearly with a range of stakeholders and staff, be able to build and maintain good working relationships someone who has the ability to influence decisions to ensure an integrated offer for the children and young people.

Central to the role is the ability to work in co-production with children/young people and their families both at an individual and system level working closely with the parent carer forum (PaCC) to ensure that the keyworking service is led by the needs and priorities of the young people and their families.

**Additional information**:
Your starting salary will be pro rata if the above position is less than 37 hours or term-time only.
