Intensive Caseworker (Complex Lives)

This is an exciting opportunity to help break the cycle of homelessness which some individuals experience. The Case Worker will provide intensive support to a small caseload of highly complex and entrenched individuals, with multiple needs including; repetitive periods of homelessness, significant histories of trauma, poor physical and/or mental health, offending behaviour, substance misuse/addiction and family breakdown. They will also have struggled to engage with other support services which exacerbates and prolongs their homelessness experience and results in them ‘revolving’ around the system.

You must be able to provide ensure effective ongoing case management and relationship building with clients and work to The Welcome Organisation ethos of low threshold and high tolerance services; all our services are based on the principles of harm reduction.

You will have a driving licence and full access to a car insured for business purposes.

**Job Purpose**

To ensure effective case management for a caseload of approx. 7-10 highly complex and entrenched individuals who will have complex and multiple needs including; repetitive periods of homelessness, significant histories of trauma, poor mental and physical health, offending behaviour, substance misuse and family breakdown.

The primary role of the post holder will be to act as a trusted key worker and to ensure effective coordination of a wrap-around support plan for each client which will have been developed through the Belfast Complex Lives Multi Disciplinary Team (MDT) process.

**Salary **:£25,000

**Hours/Days of Work **:37.5 hours per week. This comprises of a 40 hour week with a daily 30 minute unpaid lunch break. Normal working hours are 9am to 5pm (working outside of normal hours may be required in line with business needs).

**Holidays **:22 days per annum plus Public/Bank Holidays (or pro rota) and Pension Scheme

**Salary**: £25,000.00 per year

Ability to commute/relocate:

- Belfast: reliably commute or plan to relocate before starting work (required)
