The Senior Practitioner will work as part of the multi-disciplinary community support team, and will be expected to work autonomously, managing a defined caseload of adult service users with learning disabilities, complex health needs., neurodevelopmental difficulties and /or challenging behaviour.

An excellent understanding of the issues adults with learning disabilities may experience is essential for this post, as is a good understanding of Positive Behaviour Support, Mental Health, Positive Risk Assessment, Care Programme Approach and Case Management.

In addition to direct clinical skill, the successful applicants will be able to demonstrate skills in sharing knowledge through training and consultation and be able to modify and evaluate practice in line with clinical and service needs.

The post holder will be an excellent communicator, have good time management skills and demonstrate a willingness to be an effective and robust team player. They will also need to demonstrate an ability to work across a range of health and social care settings with a clear emphasis on the development and training of others. They would be expected to lead on Quality Improvements initiatives and projects that will have a significant impact on the health and wellbeing of people with a learning disability across East & South Cheshire.
