Rough Sleeper Navigator

To support rough sleepers in Newton Abbot area to transition to sustainable housing and reduce rough sleeping, implementing the client's Homelessness Strategy.

**Key Responsibilities**:

- Conduct street outreach and hot spot visits to engage rough sleepers.
- Perform assessments and develop support plans for clients.
- Coordinate with housing, health, and support services for positive outcomes.
- Assist clients in accessing services, social networks, and meaningful activities.
- Facilitate multi-agency collaboration to address street-based activities.
- Overcome housing barriers and promote client involvement.

**Required Skills/Experience**:

- Experience: Partnership working, homeless support, assessment, and planning.
- Skills: Driving, clear communication, resilience, IT proficiency, record-keeping.
- Knowledge: Homelessness legislation, issues faced by rough sleepers, benefits system.
- Aptitude: Mobility, customer care, equality principles, adaptability, empathy.
- Commitment to equality and diversity.
- Diplomatic approach, positive health promotion, and good attendance record.
- Full UK driving licence required
- Unsociable hours (Usually once - twice a week to oversee outreach)

**Salary**: From £15.98 per hour
