**About the role**
To provide support to young people aged 18-25 with mental health or low-level learning disability issues, who are being discharged from the University of Durham Hospital and have accommodation to return to. This could be their own accommodation of any tenure or they could be living in with family or friends. The main aim of the service is to reduce reoccurring presentations at both GP practices and A&E departments.
- Support will include:

- Being a named contact person
- Support to maximise income and manage tenancy as required
- Support to access and engage with primary care and other related health services
- Support to access training, employment and education opportunities
- Signposting to / support to access other partner agencies / specialist services
- Support to reduce isolation
- Assistance to attend appointments where required
- Building networks within the community
- Coordination of a support plan
- Facilitating access to a personalisation pot if required
- Support for coordination and delivery of multi-agency meetings as required
- This role will be based in our head office at Bowburn and will work closely with in-hospital support service and discharge teams within the hospital._
