**PRIMARY PURPOSE OF THE JOB**:
Responsible for providing high-quality Early Help support that combines outreach and home-based services, using a whole-family approach (levels 2 and 3). Our aim is to deliver targeted support to children of all ages (0-25*) and their families, aiming to prevent the need for statutory services and ensuring families access Universal services, with a focus on those between ages 19-25 where the SEND Code of Practice applies.

**MAIN AREAS OF RESPONSIBILITY**:
Manage complex cases that transfer from children's social care (level 3/2), including conducting early help assessments, organizing Team Around the Family meetings, and assuming the role of Lead Professional to improve outcomes for children. Also, provide support on level 4 cases to ensure a smooth transition between early help and social care. Offer bespoke packages and programs to support vulnerable children and adolescents with complex emotional and mental health issues. Conduct risk and need assessments through early help assessments on Careline cases and adopt the Lead Professional role for children and families requiring more support than universal services.
Develop Team Around the Family plans to reduce risk and need, and coordinate professionals and services to implement and review the plan. Use de-escalation methods to reduce challenging behaviour from adults and children. Align work with the Signs of Safety (SOS) model and use the Family Outcomes Star to gauge progress towards positive outcomes. Facilitate meetings between family members to develop practical parenting strategies that enhance positive parent-child relations.
Record effective plans, assessments, chronologies, and general notes on electronic systems. Attend regular supervision sessions with the Consultant Social Worker and provide evidence of improving outcomes for children and young people. Visit children, young people, and families in their homes to provide targeted support. Adhere to Early Help Hub Practice Standards and support supervised contact for open cases. Promote equal opportunities, safeguard children and young people, and carry out other duties as necessary. Use Information Technology and attend staff meetings, reviews, planning meetings, and conferences.
