We currently have an exciting opportunity for a Band 4 Nursing Associate to join the community nursing team in Chesterfield. This role will work directly alongside an Advanced Community Practitioner providing care for patients with long-term conditions in the community.
As a Nurse Associate you will be expected to provide community based individualised care to include; long-term condition management, venepuncture and medication administration as well as completing various nursing assessments and following treatment regimes.
You will be based within the GP practice and you will provide nursing care to patients in their own homes within a designated area.
This is a very exciting new role and the successful applicant will really develop their long-term condition management skills whilst working closely with the GP practice.
Working as an Nurse Associate you will need to be passionate and enthusiastic about patient care putting patients at the heart of everything you do.
You will be working in a multi-professional team and will need to be able to think on the spot and be empathetic.
For this role you will need to be able to work as part of a team and communicate effectively with other staff and patients. You will also have the opportunity to manage your workload on a daily basis, following the guidance of your line manager.
The post holder will organise and prioritise their own day to day work tasks and activities, and delegate tasks to junior colleagues. The successful post holder must be able to able to work alone and unsupervised.
The post holder will be required to have access to a car to complete patient visits in the community and clinical settings.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

The Nursing Associate has a breadth of knowledge across the lifespan and the four fields of nursing, providing holistic and person centred care and support for people of all ages and in a variety of settings.
The Nursing Associate works independently under the supervision of Registered Nurses, working within the sphere of nursing and care and within all aspects of the nursing process.
Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a Nursing Associate.
Nursing Associates will adhere to the Nursing and Midwifery Code of
Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency for nursing Associates.
The Nursing Associate will contribute to the health and wellbeing of our patients.
Integral to this role is the ability to communicate effectively, with sensitivity and compassion and to manage relationships with people,
making reasonable adjustments where necessary.
The overriding purpose of the role of Nursing Associate is to support the provision of the highest quality patient care through personal actions and continuous improvement.
The post holder will support the Registered Nurses within their team in ensuring the delivery of high quality patient care, upholding dignity, delivered with kindness and compassion.
The post holder will provide and monitor the nursing care for a designated group of patients/clients during a care episode.
They will actively contribute to an effective learning environment and support others in their learning.
The post holder will promote a team nursing philosophy of care.
