**Salary**:£26845 FTE of 37 hours (pro rata for part time posts)

**Hours**:37 hours per week

Monday - Thursday 10am -6pm

Friday’s 9:30am - 5pm

**Contract Type**:Fixed Term until 31st March 2024

**Travel expenses**:Public transport at cost or 45p/mile (The use of a car and full driving licence will be required for this role.)

**Responsible to**:DHFHSS Field Partner Manager

**Based**:The role involves home working and work in the community in a specified geographical area.

South Derbyshire CVS are delighted to be recruiting staff for a pilot project that will support discharges from hospital to help ease winter pressures on our hospitals. Working alongside the existing Derbyshire Home from Hospital Support Service team this is a great opportunity to make a difference. You will join a team working across Derbyshire and be supported in this new role. We are open to considering secondments and job sharing. The positions we are now recruiting for are:

- 1 x Hospital Discharge Support Coordinator (South Derbyshire and Erewash)
- 1 x Hospital Discharge Support Coordinator (Chesterfield, NE and Bolsover)
- 1 x Hospital Discharge Support Coordinator (South Dales and Amber Valley)
- 1 x Hospital Discharge Support Coordinator (North Dales and High Peak)
- 1 x Home from Hospital Project Support worker.

**Purpose of job**

The Hospital Discharge Support Coordinator Role is part of a pilot, working alongside our existing Derbyshire Home from Hospital Support Service, to further facilitate smooth and timely discharge of patients from hospital back to their own home. The Hospital Discharge Support Coordinator will respond to requests from discharge teams and other DHFHSS staff at acute hospitals to support an individual to return home. Responding to requests within specified geographical areas the coordinator will assess need, deliver and review a service response aimed at facilitating discharge from hospital and coordinating support to enable the person to settle back into their home environment safely.

This role will involve regular travel with occasional transportation of service users.

**Main duties**
- To liaise with Hospital Discharge Teams and Derbyshire Home from Hospital based coordinators regarding any patients being discharged from hospital.
- To gather all relevant information regarding the referral of an individual to facilitate a safe and timely discharge from hospital.
- Establishing whether the service user will require a meeting at home once discharged for a settling visit or contact by telephone.
- To gather any risk information from referrers prior to discharge and carry out appropriate risk assessments when home visiting.
- To liaise with HFHSS service users and those referring to the service to establish eligibility; where appropriate, to undertake and document a person-centred holistic assessment of need.
- To oversee the implementation of a person-centred delivery plan in response to the assessed need, linking with other community-based services where appropriate.
- To provide immediate support in the service users home in terms of providing a meal deal/ simple shopping on arrival home from hospital and plan for support with shopping for 6 weeks if needed.
- To ensure the home is warm safe and comfortable and refer to other services if support with this is required.
- Provide ongoing support for 6 weeks or refer on to District Coordinator for the Derbyshire Home from Hospital Service.
- To review service user progress at planned intervals, involving service users, family and referrers as appropriate; to work towards an agreed exit of the service.
- To record all activities and keep up to date and accurate case notes on CRM.
- To produce service and monitoring reports.
- To work within agreed service standards and ensure contractual targets are met.
- To comply with all organisational policies and procedures and be fully committed to safeguarding, confidentiality and equality & diversity when carrying out duties.
- To contribute to the overarching aims and work of the organisation as required.

To support the above work the post holder will be expected to:

- Monitor their work, keep accurate records and produce reports and statistics as required;
- Attend staff and team meetings, staff training and organisational development days and the Annual General Meeting;
- Liaise with staff in partner agencies;
- Be responsible for their own health and safety and the health and safety of others who may be affected by their acts or omissions at work;
