We have an amazing opportunity for Discharge Coordinator to join our caring and support team at our Hothfield Brain Injury Rehabilitation and Neurological Care Centre, our specialist centre in Kent, we look after adults with a brain injury or other neurological condition.

Set within the 12 acre grounds of a manor house, we specialise in intensive rehabilitation for people in the early stages of recovery from a brain injury right through to residential care.

**What you'll be working**:
FULL TIME - 18.75 hours per week

Flexible, ideally working across Monday to Friday

**What you'll be doing**:
**Overview of post**:
An exciting opportunity has become available at Hothfield Neuro Rehab unit to work closely with the multi-disciplinary team (therapy, nursing and care staff) and other statutory and non-statutory agencies to ensure the rehab and discharge planning process is responsive to the needs of patients and their carers. The successful applicant will be a key worker for each of the 22 patients on the neuro rehab unit, supporting them and their NOK through the rehab and discharge journey.

The successful applicant will need to be confident liaising directly with family members, carers and patient who may have communication and cognitive difficulties and proficient in administrative tasks. Experience in working within a multi-agency care team in desirable, but training can be provided for someone is keen to learn.

**Key Accountabilities**:

- To be the key worker for a defined caseload of patients / service users.
- To be a key contact for family members and NOK.
- To be able to explain the rehabilitation process and discuss client expectations and goals - informal training can be provided on this if required.
- To work in partnership with service users, carers, other team members and other statutory and non-statutory agencies to disseminate key information about a service users progress and ongoing needs
- To liaise closely with other agencies at the point of discharge to ensure a smooth transition.
- Arrange and chair key meetings, taking notes and sharing these with external services as required.
- To write report templates and complete referrals - informal training can be provided on this if required.
- To ensure appropriate risk assessments and care plans are completed through audits and escalating to the relevant member of the MDT.
- To maintain contemporaneous records and comply with relevant ACG polices.
- To ensure that all relevant data is entered onto ACG information and national (e.g. UKROC) systems.
- To positively address issues of discrimination and social exclusion.
- To use the results of and participate in clinical audit and research as required.
- To participate in supervision, within the requirements of ACG policy.
- Undertaking other duties appropriate to the post as requested by management
