FIXED TERM MATERNITY COVER VACANCY UNTIL 19.07.2024

This role is to support clients in Hertfordshire who are released from prison. The succesessful applicant will meet with clients face to face in prison before release and also in person after release. There is scope for use of Teams to engage clients, but this is primarily a face to face support and signposting service.

RECONNECT aims to address the health inequalities experienced by those released from prison. The RECONNECT worker supports them to make the transition to community-based services that will provide the health and care support.

We are looking for an enthusiastic individual who will be responsible for processing client referrals and assessing and supporting client needs. The individual will need to be well organised and able to build and maintain relationships with internal and external stakeholders. You will be required to travel across the county and be flexible to service need. This is a post which requires the successful applicant to be home based and work remotely with clients as well as engage with them in person in the community and within a prison setting prior to their release.
