This is a joint role working across Mid Essex Alliance partners and hosted by Provide. (Secondments will be considered)
The postholder will provide a supporting role within the Mid Essex Integrated Neighbourhood Team. This team will be building on a vibrant social, voluntary and community sector to support individuals to look after their health and wellbeing, connect with others, manage their long-term conditions and stay independent. It will be delivered through a partnership approach, bringing together people from a wide range of organisations from health, social care, public and voluntary sector organisations.
The role will lead delivery of this approach within a defined geographical area, promoting the embedding of a culture of collaborative innovation and integration across partners, including residents.
The role will support the team to define and deliver objectives specific to the needs of the neighbourhoods, securing tangible improvements in health and wellbeing across local communities. This person will be responsible for working within the neighbourhoods to deliver the agreed operational plans.
Although the postholder will be based within the Neighbourhood team, they will work closely and collaboratively with teams across the Integrated Care System.
The postholder will be a dynamic, motivated individual who is a self-starter, able to adapt to new strategies, ideas and ways of working as they enable delivery of the integrated neighbourhood approach.
- Contribute to the delivery of one or more strategic neighbourhood plans, enabling key milestones to be delivered and raising any issues that arise in a prompt manner.
- Responsible for monitoring budget/s, contributing to budget setting and working knowledge of financial processes.
- Responsible for decisions made whilst undertaking their duties, seeking guidance and escalating issues where appropriate.
- A day-to-day point of contact for an allocated number of neighbourhoods in a defined area.
- Responsible for leading specific neighbourhood projects in a defined area, dependent upon local needs and priorities.
- Responsible for leading neighbourhood-based projects to deliver agreed strategic outcomes and objectives.
- To support the development of a system and culture for continuous improvement.
- Develop the neighbourhood-based workforce by arranging training and sharing good practice as required.
- Utilises range of project management tools, outcome measures and reports to plan, monitor and report on progress. In doing so, proactively escalates risk and supports the identification and implementation of potential solutions.
- Works in collaboration with a range of partners both within and outside of the health and care system to deliver agreed projects/workstreams.
- Works closely with the senior team and partners to understand, interpret and promote the utilisation of local population health management data to identify opportunities for projects to improve outcomes for defined populations.
