Monday to Sunday - 37.5 Hours per week (Mix of Day and Night Shifts)
£26ph Weekdays, £30ph Nights and Saturdays, £34ph Sundays and Bank Holidays

Oakwood is a registered Care Home with nursing for seven male adults with learning disabilities, autism and/or mental health needs.

**Job responsibilities**
- Participate in planning meetings; assist in the preparation of reports and present information to other professionals as needed.
- To act as an advocate for service users of the service ensuring that there is proper regard for their dignity, choice, self esteem, racial, cultural, sexual and religious needs and preferences.
- Assist and take the lead in the provision of a safe, secure and homely environment ensuring privacy, dignity and respect for service users is maintained at all times.
- Support service users to undertake household tasks, including cooking cleaning and laundry as needed.
- Assist the Manager and take the lead in the ongoing monitoring of the health (both physical and mental) and behaviour of the service users. Reporting changes to the Manager or others as appropriate and as a matter of priority.
- Ensure confidentiality in all matters relating to Service users, carers and any other issues where information has been gathered as a result of employment.
- To be responsible for assessing clients in their environment and for suitability of admission to the home.
- Responsible for assessing identified placements for suitability of clients ready for discharge.
- Ensure a high degree of environmental hygiene is monitored throughout the unit by undertaking household duties such as cooking, cleaning and laundry and directing others in these tasks.
- Understand the performance of statutory requirements and good practice guidelines affecting work, including Health and Safety at Work Act 1984, Mental Health Act 1983, Data Protection Act 1999, COSHH and Food Handling and Food Hygiene regulations.

**Main duties of the job**
- Work closely with service users and professionals from a range of disciplines in order to assess Service Users abilities and needs
- To take the lead in assessing clients/service users needs, planning and implementing and evaluating care plans and interventions designed to meet service users individual needs to enable them to live as independently as possible.
- To ensure the clients/service users are actively involved in developing and agreeing their own care plans.
- To enable all individuals living in the home to exercise choice over their own lives and ensuring that risk is balanced against benefit to the service user in relation to activities that may be available both in their environment and in the community.
- Help to develop and maintain relationships between individual service users and their families, friends, advocates and the wider community.
- Provide leadership and advice to all members of the team in the absence of the Manager.
- Ensure that agreed standard of care are implemented, monitored, evaluated and up to date records are kept.
- Ensure safe and healthy working environment by adhering to the Health and Safety regulations and Trust Policies. Ensure that the Manager is informed of all the need to repair or replace faulty and damaged equipment, furniture and fittings.
