**Dimensions of Role**: To undertake more complex interventions under the direction of the Therapist. To manage a caseload of patients referred for bed based community support. To work closely and liaise with all members of the Multi-disciplinary Team ensuring a seamless pathway of care. To ensure those patients with less complex needs received the appropriate support to be safe in their chosen environment, including the assessment for and provision of basic equipment.

To recognise the limitations of the role and seek the advice and support of the team when required. To become confident to make autonomous decisions. To support / supervise Band 3 Therapy Assistants. To work as an autonomous practitioner in the community.

To actively and effectively communicate with the patient, carers, health, social care, the voluntary and private sector agencies as appropriate. To support registered staff with more complex interventions. Knowledge and Skills: Certificate in Health Care Studies or Foundation Degree NVQ Level 4 with either having completed a recognised assistant practitioner qualification or be willing to embark on the training. Experience of working as a Therapy Assistant and be able to evidence knowledge and experience.

Able to demonstrate experience in managing a delegated workload. Experience of and be able to demonstrate the ability to use reporting and information systems. Excellent organisational and communication skills. Professional: To actively participate with team development and multi disciplinary training.

To actively participate in supervision and appraisal. To demonstrate good interpersonal skills with people using services, MDT and other agencies. Tackle discrimination and harassment, and promote equality and diversity in the workplace. Take responsibility for personal development and education and the development of a Personal Development Plan.

To carry out moderate to intense physical and mental and emotional effort throughout the working day in a highly pressurised area. Control of Infection: Be aware of, and comply with, all Trust infection prevention and control policies, to include hand hygiene, personal hygiene, environmental and food hygiene. To undertake annual training / updates inline with infection prevention and control polices. Prepare for and undertake the protective interventions that he/she is responsible for in a manner that is consistent with evidence based practice and maintaining patient safety.

**Communication and Working Relationships**: To deliver high quality responsive interventions within the service ensuring effective communication liaison with all Allied Health Professionals and outside agencies e.g. Medical staff, nursing staff and other AHP services. Clinical Responsibilities: To assist in the development of knowledge, ideas and work practice within the service and work area. Communicate and exchange condition related information with patients / carers.

To monitor the progress of patients receiving interventions and communicate changes back to the team. To deal sensitively with patients who have high levels of anxiety and aggression caused by pain, dementia, or limited mobility in frequently distressing or emotional circumstances, providing care for distressed patients / carers or clients with challenging behaviour. Following assessment advice patients of appropriate adaptive techniques. Prescribe appropriate equipment and walking aids to enable patients functional safety and mobility.

Promote dignity privacy to all patient groups. Recognise complex cases beyond own remit and pass onto the team for intervention. Liaise with and make referrals to other agencies in response to identified needs ensuring a smooth handover of care, as appropriate. Maintain records as required for patient care, as per Trust policy.

Support the work of the band 3 staff within the team and ensure care is coordinated as per the agreed management plans. To support the team in their involvement in audit and collection of statistical information. To actively participate in clinical supervision. To comply with the Trusts Moving and Handling Policy and local therapeutic handling guidance at all times.
